California Privacy Notice

This notice supplements our Privacy Policy and applies to California residents under the California Consumer Privacy Act as amended by the California Privacy Rights Act (“CCPA”). Note that medical information governed by California's Confidentiality of Medical Information Act (CMIA) or HIPAA is subject to those laws rather than the CCPA.
1. Categories of personal information we collect
- Identifiers — name, email, phone number, delivery address, government ID (for identity verification);
- Sensitive personal information — health information you provide in our intake (conditions, medications, measurements), identity-verification photos/videos, and log-in-adjacent data. We use sensitive information only to provide the healthcare services you request, and not for inferring characteristics or for advertising;
- Commercial information — the plan you select and your transaction history;
- Internet activity — device, browser, and site-usage information.
We collect this information directly from you and from your interactions with the Services, for the purposes described in the Privacy Policy. Retention periods are described there as well.
2. No sale or sharing
We do not sell your personal information, and we do not share it for cross-context behavioral advertising. If that ever changes, we will update this notice first and provide the required opt-out mechanisms before any such activity begins. We have no actual knowledge of selling or sharing personal information of consumers under 16.
3. Your California rights
- Right to know what personal information we collect, use, and disclose;
- Right to delete personal information, subject to legal retention obligations (for example, prescription records);
- Right to correct inaccurate personal information;
- Right to limit use of sensitive personal information to the services you request (which is already our practice);
- Right to non-discrimination for exercising any of these rights.
4. How to exercise your rights
Email support@zeviarx.com with the subject line “California Privacy Request.” We will verify your identity using the information associated with your account and respond within 45 days. You may use an authorized agent by providing signed permission.
