This Privacy Policy explains how Zevia Rx, Inc. (“Zevia Rx,” “we,” “us”) collects, uses, shares, and protects information when you visit zeviarx.com, complete our health quiz, purchase a treatment plan, or otherwise use our services (together, the “Services”). By using the Services you agree to this Policy. If you do not agree, please do not use the Services.
1. Who we are — and who your providers are
Zevia Rx is a telehealth platform. We connect you with independent, licensed healthcare providers who perform your consultation and make all medical decisions, and with state-licensed pharmacies that dispense and ship any medication your provider prescribes. Your treating providers and dispensing pharmacies are healthcare entities regulated under federal and state law, including, where applicable, the Health Insurance Portability and Accountability Act (“HIPAA”), and they maintain their own privacy practices with respect to your medical records. This Policy covers the information handled by the Zevia Rx platform itself.
2. Information we collect
- Contact and identity information — name, email address, phone number, state of residence, delivery address, and date-of-birth or government-issued photo ID when required to verify your identity for telehealth care.
- Health information — your quiz answers, including height, weight, BMI, medical conditions, medication history, and pregnancy status; identity-verification photos or videos required by certain states; and information generated during your care.
- Order and payment information — the plan you select and your transaction history. Card and financing details are processed directly by our payment partners (Stripe, and financing providers such as Klarna, Affirm, or Afterpay) and never touch or persist on Zevia Rx systems.
- Technical information — device, browser, and usage data collected when you use the site. Quiz progress is temporarily stored in your own browser so a page refresh does not lose your answers; you can clear it at any time through your browser settings.
3. How we use information
We use your information to provide the Services: to route your intake to a licensed provider in your state, to verify your identity as required by telehealth law, to process payment for the plan you select, to arrange pharmacy fulfillment and cold-chain shipping of any prescribed medication, to provide customer support, to communicate with you about your order and care (including the program emails and texts you agree to at sign-up, which you can opt out of at any time), to improve the Services, and to comply with law. We do not sell your personal information, and we do not use your health information for third-party advertising.
4. How we share information
- With the independent licensed providers who review your intake and provide your care;
- With the licensed pharmacies that fill and ship your prescription, and their shipping carriers (delivery details only);
- With payment and financing partners to process the transaction you request;
- With service providers that host and secure our infrastructure, under contracts limiting their use of your data to providing services to us;
- When required by law, to respond to lawful requests, or to protect the rights, safety, or property of our patients, the public, or Zevia Rx; and
- In connection with a corporate transaction (merger, acquisition, or asset sale), subject to this Policy.
5. How we protect information
All information is encrypted in transit (TLS) and at rest. Identity documents and verification videos are stored in a private, access-controlled repository that is not publicly reachable; access is limited to systems and personnel that need it to deliver your care. Payment card data is handled exclusively by our PCI-DSS-compliant payment processors. No method of transmission or storage is 100% secure, but we work to protect your information with administrative, technical, and physical safeguards appropriate to its sensitivity.
6. Retention
We retain personal information for as long as needed to provide the Services and as required by law — medical and pharmacy records are subject to state-law retention periods maintained by your providers and pharmacies. When information is no longer required, we delete or de-identify it.
7. Your choices and rights
- You may opt out of marketing emails and texts at any time via the unsubscribe link or by replying STOP.
- You may request access to, correction of, or deletion of your personal information by emailing support@zeviarx.com. We will honor your request except where we must retain information by law (for example, records of prescriptions issued).
- California residents have additional rights described in our California Privacy Notice.
8. Children
The Services are for adults 18 and older. We do not knowingly collect information from anyone under 18; if you believe a minor has provided us information, contact support@zeviarx.com and we will delete it.
9. Changes and contact
