Telehealth Informed Consent

This Telehealth Informed Consent describes what it means to receive care through telehealth on the Zevia Rx platform. By checking the agreement box in our intake and using the Services, you acknowledge and consent to the following. You may withdraw consent at any time by stopping use of the Services and notifying support@zeviarx.com — withdrawal will not affect care already delivered.
1. What telehealth is
Telehealth means receiving healthcare services remotely using electronic communications — here, a structured health intake reviewed asynchronously by an independent, licensed provider in your state, with follow-up by secure message, and, where required by your state or by clinical judgment, synchronous or recorded video interaction. Your provider may determine that telehealth is not appropriate for you and refer you to in-person care.
2. The provider relationship
The providers who review your intake are independent licensed clinicians, not employees of Zevia Rx. They exercise independent medical judgment, are licensed in the state where you are located, and are solely responsible for all clinical decisions — including whether to prescribe any medication. Purchasing a plan does not guarantee a prescription. If your provider determines treatment is not appropriate, your medication cost and shipping fee are refunded under our Refund & Cancellation Policy; the provider consultation fee is non-refundable.
3. Benefits and risks
Benefits of telehealth include convenient access to licensed clinicians without travel or scheduling delays. Risks include: your provider does not perform a physical examination and relies on the information you provide, so incomplete or inaccurate answers can lead to inappropriate care; delays can occur due to technology failures; and, despite safeguards, electronic transmission of information carries security risks (see our Privacy Policy). Alternatives to telehealth are available to you at any time, including in-person visits with your own physician.
4. Medication-specific disclosures
GLP-1 medications carry significant risks, including a boxed warning regarding thyroid C-cell tumors; they are not appropriate for people with a personal or family history of medullary thyroid carcinoma or MEN 2, and carry other risks your provider will review with you. Where compounded medication is prescribed, it is prepared for you by a state-licensed compounding pharmacy and is not FDA-approved; the FDA does not review compounded drugs for safety, effectiveness, or quality. Report side effects to your provider promptly; for emergencies call 911; adverse events can also be reported to support@zeviarx.com and FDA MedWatch.
5. Identity verification and recordings
Telehealth law requires verification of your identity. You agree to provide a government-issued photo ID and, in states that require it, a short self-recorded video confirming your identity and consent to treatment. These materials are stored securely as part of your care record.
6. Your rights
You have the right to: ask questions about your care and treatment options; refuse or discontinue treatment at any time; request your records; know your provider's name and licensure; and receive care without discrimination. Your medical information is handled as described in our Privacy Policy and your providers' own privacy practices, and is protected by applicable federal and state law.
7. Consent
By proceeding, you confirm that you are at least 18 years old, that you are the person receiving care, that the information you provide is true and complete, and that you consent to receive care through telehealth as described above.
